Health Savings Account (HSA) Terms and Conditions

Anthem is your Administrator for this Health Savings Account Program. Your specific HSA Program may be referred to or branded as Anthem Blue Cross Blue Shield, Anthem Blue Cross, Empire Blue Cross Blue Shield or Empire Blue Cross. Please refer to your Welcome Materials for more information. References to your Administrator throughout this HSA Program experience will collectively mean Anthem.

As part of the Health Savings Account Program established by Anthem, your Administrator, you are being provided with the HSA Terms and Conditions that govern the use of your HSA. PNC Bank, N.A. is the Custodian of your HSA. Please open and read through each document carefully. You should print and keep a copy of these documents for your records.

PLEASE NOTE: The Custodian of your HSA, PNC Bank, N.A. does not assess any fees associated to this HSA Program. Should you have any questions regarding the fees associated to this HSA that are being assessed by your Administrator please contact your Administrator using the phone number on the back of your debit card or listed on your quarterly account statement.

Important Information about Procedures for Opening a New Account

To help the government fight the funding of terrorism and money laundering activities, federal law requires all financial institutions to obtain, verify, and record information that identifies each customer who opens an account.  What this means for you: When you open an account, we will ask for your name, address, date of birth and other information that will allow us to identify you. We may also ask to see your driver’s license or other identifying documents.

Your online registration and establishment of your username and password and ongoing use of this account will constitute as your receipt of, and agreement to these Terms and Conditions.

PNC Bank, National Association is the Custodian of your Health Savings Account (HSA).

Banking and lending products and services, bank deposit products and treasury management services for clients and/or customers are  provided by PNC Bank, National Association, a wholly-owned subsidiary of PNC and Member FDIC.

Mutual funds are selected by Devenir Investment Advisors, LLC. Devenir Investment Advisors, LLC is not an affiliate of PNC Bank. Shares of mutual funds are not deposits or obligations or guaranteed or endorsed by any bank, nor are they federally insured or otherwise supported by the FDIC or any other governmental agency, and may lose value. Investments in mutual funds involve risk. The prices of shares of mutual funds fluctuate and when redeemed, your shares may be worth more or less than their original cost to you. Please read your HSA Custodial Agreement and Privacy Policy and the HSA Investment Terms and Conditions shown above. In addition, please carefully read the applicable prospectus for each mutual fund before you invest or send money. The prospectuses contain more complete information on the mutual funds, including underlying fees, expenses, investment objectives and risks. Some funds may impose a redemption fee under certain circumstances.

PNC Bank, National Association has been selected by your Administrator to act as Custodian of your Health Savings Account and PNC Bank does not select the mutual funds available through your Administrator’s platform.
