Upon receipt of this form, Clover Care protection for your Clover device will be added to your account. Your coverage will be effective as of the date submitted and should reflect on your account within 3 business days. The entire payment owed will be applied within the next two billing cycles depending on the date submitted.

Important Legal Disclosures and Information

  1. If we can’t help you with your Clover device over the phone, then we will send you a new Clover device. Next-business-day delivery requests received before 3:00PM ET the daily cut-off time and will generally ship the same business day. Otherwise, they ship the next business day. Please contact the Help Desk, if you have any questions about your specific daily cut-off time. The three-year term is based on initial ship date of the device. Clover Care protection includes the following: manufacturer defects; cracked/broken screen; environmental conditions (dust, dirt, etc.); liquid damage to unit or printer in all states except for NY. (For NY merchants, Clover Care covers manufacturer warranty defects). Clover Care does not cover intentional damage, theft, loss or fire damage.

    For NY merchants, Clover Care only covers manufacturer warranty defects.

  2. Purchase fees apply. The price of Clover Care varies based on equipment type.
